Care Coordination Team offer a range of interventions to support clients through their treatment journey. We do this by initial assessment of client needs which then formulates specific care plans tailored to each individual. We aim to provide structured support, day to day, week to week.

Redcar Care Coordination Service will work with individuals over the age of 18 years with a substance misuse problem. Individuals can self refer or be referred through the Criminal Justice System or other treatment providers.
The service can be accessed by ‘drop in’ or appointment. Individuals can self refer or be referred from the following agencies:
Each individual will be seen by a member of the care coordination team. This worker will remain with the individual throughout their treatment journey.
The worker will undertake an initial / triage assessment to assess individual needs.
The service can offer structured support to develop a care plan which will suit your needs and help you address the issues surrounding your substance misuse.
