ReNew: Criminal Justice Service – Hull
Case Managers (Treatment)
ReNew offers end to end support for service users with alcohol or drug issues and their families who are involved in the Criminal Justice System. This includes prevention and engagement, early intervention, case management, medically assisted recovery, family interventions, referral into and awareness of 12 Step and CBT based group work and all aspects of aftercare. To deliver this contract we are also utilising the expertise of RAPt and Work Solutions.
We are seeking committed, experienced, outcomes focused and enthusiastic individuals to provide a comprehensive case management function for service users in structured treatment (Social Work Qualification desirable).
Whilst the ReNew Service is committed to early engagement and a proactive approach to reducing the length of time service users are engaged in services, we recognise that for some service users with complex needs a longer intervention is required. The aim is for all service users to successfully exit treatment within 2 years.
As a case manager you will support every element of a service users’ journey through treatment and their holistic recovery plan. This will include delivering direct interventions such as assessment, recovery planning, and utilising skills such as motivational interviewing, ITEP, cognitive behavioural techniques and brief solution focused therapy to assist service users to explore, understand and address substance use, offending and social functioning. The role will also include facilitating access to medically assisted recovery, health and well-being interventions and group-based programmes on offer within the service. It will involve supporting service users to plan and achieve abstinence and to develop a robust aftercare plan designed to promote long-term sustainable change. You will be expected to proactively encourage and enable service users to involve families and carers in their recovery and to support service users to address wider needs relating to physical health, mental health, social functioning, housing, finances, family, education, training and employment.
You must be highly organised and effective in managing your own caseload, including ensuring all service user care is structured, regularly reviewed and there is a clear plan for progression through treatment. You will be responsible for keeping records to a high standard and undertaking all administrative functions relevant to your post.
